Incident Details

The overdose was discovered on Sunday, July 6, 2008, after what the hospital thinks was a mix-up at the facility's pharmacy. One of the 17 babies died. However, Dr. Richard Davis, chief medical officer for the hospital, stated that the baby that died was seriously ill and the hospital is unsure as to what role, if any, heparin played in the baby's death.CNN: Up to 17 babies given overdoses of blood thinner (July 9, 2008)
